How do I know if my Vauxhall Insignia needs a brake replacement?

I have noticed that my braking has become less responsive and there is a strange noise when I press on the brake pedal. Is this a sign that I need to replace my brakes? I am concerned that this could be a safety hazard and I'm not sure when to get them checked.
