What could be causing my Vauxhall Astra to stall at low speeds?

I have a 2004 Vauxhall Astra and it has been stalling out at low speeds, usually when I'm coming to a stop sign or traffic light. It seems to be worse when the engine is warm. Any advice on what could be causing this issue?
